Mail-in Registration Form For Health and Safety Classes
If you would like to mail or fax your registration in, please click here to download a registration form.
Please keep in mind the following as you prepare to fax or mail in your registration:
- Paying For Your Class:
- Prepayment of course fee(s) is required.
- You can pay by check or money order when mailing in your registration.
- All faxed in registrations require credit card
payment.
- Obtaining A Receipt And Confirmation:
- If you mailed in the form you can include a self-addressed, stamped envelope and we will mail your confirmation and receipt back to you.
- If you faxed in your registration you can use your fax machine's transmission receipt as confirmation or mail in a self-addressed, stamped envelope and a confirmation and receipt will be mailed back to you.
- General Course Registration Information:
- Two (2) full working days notice, prior to the class, are required to apply for a refund or transfer. Late arrivals may not be admitted and course fees will be forfeited.
- A $15 processing fee will be charged for all refunds. No refunds or transfers allowed with less than two (2) full working-days notice.
- Note: Students who register within the two working days prior to the class will be committed to the class and there will be NO Refunds or Transfers allowed.
- Because of class size restrictions, groups, organizations and companies may be limited to three (3) people per class.
- A copy of the current certification must accompany
registration for review courses or where required.
